Help When You Leave the Hospital


When going home from the hospital, people with new illnesses or worsening of their long-term conditions will have changes in their health care needs. These may include medications, treatment, and follow up care from health care providers in many settings. These changes can be confusing and challenging to handle without assistance.

Transitioning from Hospital to Home
